The original poster should take a picture of the heat sink after disassembling it. Vicor's heat sink is not a simple flat plate, and has pits milled towards the transformer and other parts.

VICOR is a good product, but the heat sink glue was not removed cleanly.

It feels like a forward pulse, and the reset seems to be a direct winding reset, not an active reset architecture.

Vicor's various power modules are really good in every way, except that they are a bit expensive!
