Does the package library file .lib in protel99 have to be placed in the .ddb file to be loaded?

If you want to work overtime at home or use it elsewhere, you must package it into the .ddb file. Generally, your own proprietary library files are included with the .ddb file. If the machine is used exclusively for PCB and does not need to work overtime, you can put it anywhere.
